Mount Horeb Senior Center

The Mount Horeb Senior Center provides Case Management, MA Case Management, Nutrition Services, resources, and referrals to assist people 60+, to help them remain independent and safe in their own homes or apartments.

We serve seniors in the Villages of Mount Horeb and Blue Mounds, and the Towns of Blue Mounds, Perry, Primrose, and Springdale. We are one of fourteen agencies under the Area Agency on Aging of Dane County and Dane County Human Services.

The Southwest Dane Outreach Board consists of a representative from each municipality and a Senior Representative. Our Board meetings are scheduled for February, April, June, August, October, and December each year. View meeting dates, agendas, and minutes.

Congregate meals are served Monday, Tuesday, Thursday, and Friday at the Mt. Horeb Senior Center at 11:15 am. Congregate meals are also served every Wednesday at Finks Café from 11 am to 1 pm (Reservations are not needed for Finks Café.) Home-delivered meals are sent out Monday through Friday to all municipalities that we serve. The food for our meal program is prepared at Consolidated Foods in Verona. The suggested minimum meal donation is $4.50 per meal. We offer social events like euchre, bingo, lunch outings, shopping trips, crafts, and free tax preparation. We offer wellness programs like tai chi, blood pressure clinics, foot clinics, health education, etc. We work in conjunction with the local clinics, nursing homes, food pantry, and clothes closets.

The Case Management Services can include assisting seniors with Housing, Transportation, Meals, Energy Services, Food Share, and The Wisconsin Senior Prescription Drug Plan, and guiding them through Medicare, Social Security, and Medicare D Open Enrollment. We work with Adult Protective Services of Dane County in cases of Elder Abuse, which can include self-neglect, and financial and physical abuse. We provide donated adaptive equipment like walkers, wheelchairs, bath benches, raised toilet seats, commodes, and canes through our Loan Closet, at no cost for short and long-term use.

Transportation is provided by Care Van Services of Mt. Horeb. The cost to ride the van to the Congregate Meal Sites at the Senior Center or Finks Café is $1 per round trip, to the grocery store on Wednesday (which is Senior Discount Day) is $2 per round trip and for lunch outings or shopping trips, the cost is $3 per round trip. We work in conjunction with RSVP of Dane County to provide rides to medical appointments for people 60+. Seniors can contact the Senior Center for more information since there is paperwork that needs to be completed by the passenger before receiving the ride.

Special News & Notes

Medicare D Open Enrollment will run from October 15 through December 7, 2022. Seniors have the chance to review their Medicare D Prescription Drug Plan and check on the cost, to see if their medications are covered or if new medications are covered. Call the Mt.
